1. Purpose of Services I understand that the services provided by Eddie Mendez may include sound healing, Reiki, energy balancing, or related holistic wellness practices. These services are intended to promote relaxation, stress reduction, energetic balance, and overall well-being. I understand that these services are complementary in nature and not a substitute for medical diagnosis, treatment, or therapy. 2. Scope of Practice Practitioners do not diagnose medical conditions, prescribe medications, or perform medical treatment. I have been advised to consult with a licensed physician or healthcare provider for any physical, psychological, or medical concerns. If I am currently under medical care, I understand that it is my responsibility to continue following my healthcare provider’s recommendations. 3. Acknowledgment of Risks I understand that: Sound and energy healing sessions are generally safe, but responses vary from person to person. Possible effects may include emotional release, deep relaxation, mild fatigue, or temporary discomfort as the body processes energy shifts. I will inform the practitioner of any medical conditions, injuries, or sensitivities (including sound sensitivity, epilepsy, pacemakers, or pregnancy) before the session. 4. Voluntary Participation I voluntarily choose to receive services and acknowledge that participation is my personal choice. I may pause or stop a session at any time. 5. Confidentiality All information I share with my practitioner will be kept strictly confidential, except where disclosure is required by law. 6. Liability Release I release, indemnify, and hold harmless Eddie Mendez Sound and Reiki & Christopher Mendez Psychic Medium LLC from any and all claims, liability, or damages that may arise from my participation in these services. I acknowledge that I am solely responsible for my physical, emotional, and mental well-being during and after sessions. 7. Consent & Agreement By agreeing to this form, I confirm that: I have read and understood this Consent & Waiver Form. I have had the opportunity to ask questions and receive clarification. I understand the nature of the services and agree to participate voluntarily.
